What are rates for receiving U.S. calls inside your calling area and outside?

In the good old days, all incoming calls were free, but it seems now, depending where those calls are coming from and where you happen to be at the time you receive them there can be charges. Specifically curious about receiving US calls. I think if you are inside your local calling area (LCA) is one rate, and if you are outside your LCA it's another. Anyone know for sure?

It’s interesting to see this kind of thing because there are so many factors that you have to consider. I know that there are some things that can be frowned upon but there are just some changes to communication methods.
