You can do it yourself by downloading the latest version of iTunes, which will then walk you through the entire procedure, e.g. which rate plans you want to pay for. Yes, the iPhone AT&T pans are now available beginning with $59.99 for 450 minutes of calling. The plan includes “unlimited data, Visual Voicemail, 200 SMS text messages, roll-over minutes and unlimited mobile-to-mobile calling.” There’s also a family plan if you want to buy relatives an iPhone.
